In contrast with BAL, both DMSA and DMPS show lower toxicity and are effective when administered orally. Many clinical trials have proved DMSA safe and well tolerated by patients. On the whole, the efficacy as antidotes is the same for both DMSA and DMPS. However, the LD_50 of DMSA in mammals is almost 3 times greater than that of DMPS. In arsenic poisoned rats, DMSA promoted excretion of arsenic via urine, while DMPS eliminated the poison through the bile. This difference in the route of elimination accords with pharmacokinetic studies showing that DMSA is distributed mainly extracellularly, and DMPS mainly intracellularly. These contrasting distributions explain the lower toxicity of DMSA compared with DMPS.
