As the first objective of treatment was the elimination of thumb-sucking, we instructed the patient in myofunctional therapy. As this was unsuccessful after a year, we installed a palatal crib, which successsfully eliminated the pernicious habit. The maxillary first premolars were subsequently extracted, and the 2nd phase of treatment was begun with edgewise appliance to correct the occlusion.
