ガミースマイルはフルスマイル時に魅力的でないと判断されてしまうことが多い。本研究ではガミースマイルを伴う矯正治療患者のスマイル時の顔貌改善を目的として、特に口唇の動きに焦点をあて、スマイル時の正常咬合者の口唇の動きとの差について三次元的に比較検討を行った。[資料および方法]本研究に同意の得られた個性正常咬合者34名（男性23名、平均年齢21.1±2.3歳；女性11名、平均年齢20.4±2.3歳）およびガミースマイルを伴う矯正治療女性患者（以下、ガミースマイル群）15名（平均年齢23.4歳±5.4歳）を対象とした。閉唇時とポーズドスマイル時およびフルスマイル時のステレオ画像を撮影し、三次元解析ソフトウェア（QM3000）を用いて、顔貌写真を立体構築した。閉唇時－ポーズドスマイル時と閉唇時－フルスマイル時の上下口唇中央部、左右口角部および左右頬部の動きについて、ステレオ画像計測法を用いて三次元的に解析し、正常咬合者内と正常咬合者群およびガミースマイル群間にて、各測定点間の差の統計処理を行った。[結果]１．正常咬合者群内閉唇時－ポーズドスマイル時と閉唇時－フルスマイル時の比較で、女性群では水平方向の左右口角部に有意差を認めたが、垂直・前後方向では有意差は認めなかった。一方、男性群では、水平方向の上唇・下唇中央および垂直方向の下唇中央を除く部位で有意差を認め、前後方向では有意差は認めなかった。２．正常咬合者群とガミースマイル群閉唇時－ポーズドスマイル時と閉唇時－フルスマイル時の比較で、水平方向では正常咬合者群とガミースマイル群間で有意差は認めなかった。垂直方向では正常咬合者群とガミースマイル群間でポーズドスマイル時において全ての計測項目で有意差は認めなかった。閉唇時－フルスマイル時では、ガミースマイル群の上唇中央は上方への有意に大きい運動を示した。前後方向の運動では、ポーズドスマイルおよびフルスマイルの下唇中央に有意差を認め、ガミースマイル群の下唇は後方へ有意に大きい運動を示した。[考察および結論]ポーズドスマイルは「感情に関わらない学習された自発的な笑顔で、高い再現性」が報告されている。正常咬合者における男女差について、男性は日常的に表情を意識する機会が女性と比較すると少ないと推測されることから、フルスマイル時に対するポーズドスマイル時の表情の変化は小さくなったと考えられた。また、正常咬合者群とガミースマイル群の比較において、女性は、日常的に顔や表情を意識する機会が多いと推測されることから、本研究におけるガミースマイル群の女性のポーズドスマイル時の水平・垂直方向で有意差が認められなかったと考えられた。ガミースマイルを伴う矯正治療患者のフルスマイル時には、正常咬合者と比較して大きな上唇の上方への垂直的な運動を有することが明らかとなった。従って、ガミースマイルを伴う患者の矯正治療の際は上顎前歯の圧下のみならず、上唇の上方への垂直的および下唇の後方への前後的な運動量に対する対応が治療方針決定の一要因となる可能性が示唆された。","subitem_description_type":"Abstract"}]},"item_10006_dissertation_number_22":{"attribute_name":"学位授与番号","attribute_value_mlt":[{"subitem_dissertationnumber":"甲第227号"}]},"item_10006_version_type_18":{"attribute_name":"著者版フラグ","attribute_value_mlt":[{"subitem_version_resource":"http://purl.org/coar/version/c_970fb48d4fbd8a85","subitem_version_type":"VoR"}]},"item_access_right":{"attribute_name":"アクセス権","attribute_value_mlt":[{"subitem_access_right":"open
