Hironao"}],"nameIdentifiers":[{}]}]},"item_10006_date_granted_11":{"attribute_name":"学位授与年月日","attribute_value_mlt":[{"subitem_dategranted":"2022-01-31"}]},"item_10006_degree_grantor_9":{"attribute_name":"学位授与大学","attribute_value_mlt":[{"subitem_degreegrantor":[{"subitem_degreegrantor_name":"松本歯科大学"}],"subitem_degreegrantor_identifier":[{}]}]},"item_10006_degree_name_8":{"attribute_name":"学位名","attribute_value_mlt":[{"subitem_degreename":"博士（歯学）"}]},"item_10006_description_10":{"attribute_name":"学位授与年度","attribute_value_mlt":[{"subitem_description":"2021","subitem_description_type":"Other"}]},"item_10006_description_12":{"attribute_name":"報告番号","attribute_value_mlt":[{"subitem_description":"甲第242号","subitem_description_type":"Other"}]},"item_10006_description_17":{"attribute_name":"フォーマット","attribute_value_mlt":[{"subitem_description":"application/pdf","subitem_description_type":"Other"}]},"item_10006_description_7":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"[緒言]経管栄養法は経口摂取が困難な要介護高齢者の栄養維持のために広く用いられており，日本の要介護高齢者における経管栄養の実施割合は，介護療養病床で62.2％，病院で63.3％とされている1）．経管栄養で意識障害が伴うような非経口摂取者は，常時開口がみられ，舌の低活動により，舌を介して唾液が口腔粘膜に付着せず，口腔粘膜が乾燥する．その結果，口腔粘膜や歯面に剥離上皮膜が形成されることがある2,3)．経管栄養患者において，口蓋で40.0％，舌背は42.5％，歯面は20％，頬粘膜は17.5％の者に剥離上皮膜が観察されたとする報告がある2，3)．さらに口腔に剥離上皮膜がある者のうち70％の者が咽頭粘膜上に剥離上皮膜が観察されている4)．剥離上皮膜は，重層扁平上皮由来の角質変性物と唾液のムチンが主体となり，部分的に炎症性細胞や細菌の集積を認め，痰や痂疲とは異なる5,6)．剥離上皮膜の一部は，口腔粘膜と結合しており，剥離上皮膜の除去時に出血する可能性がある7)．また剥離上皮膜が咽頭・喉頭に落下し，気道を閉塞させ，呼吸抑制や窒息となる危険性がある8,9)．さらに剥離上皮膜形成者に有意に発熱が認められ10)，経管栄養と肺炎との関連が報告11-13)されていることから，発熱原因として，除去時の出血部位からの局所感染や頭・喉頭に落下した剥離上皮膜を介する口蓋粘膜細菌の呼吸器感染が考えられる．経管栄養者の口腔内細菌叢として，舌苔からCorynebacterium 属，Peptostreptococcus 属，Fusobacterium 属が経口摂取者より有意に多く検出されたとする報告14）や，口蓋・舌背・咽頭からStreptococcus 属，Rothia 属，Neisseria属が経口摂取者と比べ有意に多く検出されたとの報告15）がある．経管栄養者の口腔内細菌叢は経口摂取者と異なり14,15)，経口摂取の有無が細菌叢に影響を及ぼすとされる15)．しかし，剥離上皮膜が存在する経管栄養者の口腔粘膜には，う蝕原因菌，歯周疾患関連菌，全身へ影響する細菌の存在は，明らかにされておらず，細菌学的為害性は不明である．今回，経管栄養の要介護高齢者の口腔内のうち，剥離上皮膜が認められることの多い口蓋を対象として，次世代シークエンス16s
